  1. Complete the intake form
  2. Check your inbox (and spam folders) for an email from the Occupational Therapist who will be providing the assessment.
  3. Complete the questionnaire
  4. OT will evaluate and score the results, then send an invoice.
  5. Once invoice is paid, report will be emailed
